Alert Summary

FM Approvals has been made aware of a falsified Certificate of Compliance (CoC) issued on FM Approvals’ letterhead. The document (attached as Figure 1) misrepresents pipe insulation for piping, ducts and chiller equipment in both tubular and sheet form manufactured by Eastern Polymer as FM Approved. Eastern Polymer has cooperated in FM Approvals’ investigation and claims to have no knowledge of the origin of the counterfeit CoC.
